This form is similar to a student's transcript of records and each student is expected to keep it up-to-date every semester.
The form is very helpful at registration time in making the proper choice of subjects to be taken and in getting the approval of the college deans.
The form is basically a checklist of all general and major course requirements for a particular degree program.
It helps the student keep track of his academic progress and thereby assumes responsibility for the completion of all requirements for graduation.
The record also serves as the basis of the student's application for graduation during his senior year.
Sequential subjects should be followed strictly. A student who has taken higher subjects but failed or who has not taken the pre-requisite subjects has to retake the subjects.
The school is not obliged to offer subjects for students who failed to follow the curriculum or who failed in their subjects.
Students should follow the subjects recommended by their college deans.
Students are advised to check with the Registrar's staff regarding discrepancies of grades in their records such as NT, NG and INC. These grades become final and cannot be changed once reported to CHED forty five (45) days after the last day of the semester.
POST GRADUATION
Proof of graduation is the Special Order. Students are advised to check at the Registrar's Office for deficiency in their records, such as failing grades in the last term before their graduation.
