This Form is similar to a student’s transcript of records and each student is expected to keep it up-to-date every semester.  The form is very helpful at registration time in making the proper choice of subjects to be taken and in getting the approval of the College Deans.  The form is basically a checklist of all general and major course requirements for a particular degree program. 

It helps the students keep track of their academic progress and thereby assume responsibility for the completion of all requirements for graduation.  The record also serves as the basis of the application for graduation during the senior year of the students
